We appreciate that you have scheduled a dental appointment scheduled at our office, and we want to make you aware of some new steps we are taking in order to keep you and our staff safe.
- Before your appointment, fill out the Pre-appointment Form shown below. If you answer “yes” to any of the questions, please call us before coming for your appointment
- Please bring a facial covering and wear it while in the waiting room
- If all chairs are taken, please check in at the front desk, leave your cell phone number with us, and wait in your car. We will call you when we are ready to see you
- We request that only the patient comes into the office, if possible, as well as into the back rooms to decrease traffic and allow for social distancing
